ABSTRACT

Depression is a mental disorder characterized by persistent low mood and belongs to the category of "stagnation syndrome" in traditional Chinese medicine (TCM), with the characteristics of high prevalence, high disability rate, high suicide rate, and high recurrence rate. The pathogenesis of depression is extremely complex and involves factors such as genetics, psychology, and social environment. Currently, clinical antidepressant drugs such as tricyclic and tetracyclic antidepressants, and selective serotonin reuptake inhibitors, have slow onset of action and significant adverse effects with long-term application. Furthermore, clinical statistics show that about one-third of patients do not respond to these types of medications. Enhancing the effectiveness of depression treatment has become a major challenge in the medical field. TCM, based on the holistic view and treatment based on syndrome differentiation, has unique advantages in the prevention and treatment of depression, including stable therapeutic effects, low recurrence rate, minimal side effects, and good patient compliance. As one of the heat-clearing drugs, Gardeniae Fructus has the effects of clearing heat, purging fire, cooling blood, and relieving restlessness, which makes it effective in treating patients with "stagnation syndrome". Literature studies have found that active components of Gardeniae Fructus, such as geniposide, genipin, and crocin, as well as drug pairs such as Gardeniae Fructus-Chuanxiong Rhizoma, Gardeniae Fructus-Acanthopanacis Senticosi Radix et Rhizoma Seu Caulis, and Gardeniae Fructus-Lycii Fructus, and prescriptions such as Zhizichi Tang, Yuejuwan, Zhizi Houpotang, Danzhi Xiaoyaosan, and Jieyu Anshen Granules, have shown significant antidepressant effects. The mechanism of action may be related to regulating the hypothalamic-pituitary-adrenal (HPA) axis, modulating neurotransmitters such as serotonin (5-HT) and dopamine (DA), increasing brain-derived neurotrophic factor (BDNF) in the hippocampus, enhancing neurogenesis in the hippocampus, and regulating the nuclear factor-kappa B (NF-κB) signaling pathway to inhibit neuroinflammation. This article summarized the research progress on the active components of Gardeniae Fructus, drug pairs, and Chinse medicinal prescriptions containing Gardeniae Fructus, providing references for further promoting the clinical application of Gardeniae Fructus and its prescriptions in the treatment of depression.

ABSTRACT

Objective To explore cross-talk elimination method in the determination of gross α and gross β activities using a low-background α/β gas-flow proportional counter. Methods A CLB-104 low-background α/β gas-flow proportional counter was used in this study. First, the α threshold was increased to eliminate the cross-talk counting caused by β particles in the α channel. Then, the α-β anticoincidence threshold was reduced to eliminate the cross-talk counting induced by low-energy α particles in the β channel, and β counts were corrected to eliminate the counts induced by internal convention electrons in the β channel. Finally, gross α and gross β activities of non-saline water samples with different activity levels were determined and compared with gross α and gross β activities of the same samples determined on a BH1227 low-background α/β solid scintillation counter, in order to verify effectiveness of the cross-talk elimination method. Results By eliminating the cross-talk counts of β particles in the α channel and the cross-talk counts of α particles in the β channel, and deducting the counts of internal convention electrons in the β channel, the gross α and gross β activities of the same samples determined by CLB-104 were consistent with the values determined by BH1227. Conclusion Cross-talk counts induced by low-energy α particles or β particles can be eliminated by threshold adjustment, and the counts caused by internal convention electrons in the β channel can be eliminated by correction.

ABSTRACT

Objective:To investigate the effects of deep learning-based AiContour ??versus atlas-based Raystation ?? automatic contouring methods on the contouring of organs-at-risk on CT images of patients with rectal cancer who undergo radiotherapy, providing evidence for clinical application. Methods:Fifty patients with rectal cancer who received treatment during January to June 2020 in Zhejiang Provincial People's Hospital (Affiliated Hospital of Hangzhou Medical College) were included in this study. The CT images from 20 patients with rectal cancer that had been contoured by experienced radiotherapist were selected as target images and automatically contoured using the data template library of AiContour ?? and Raystation ?? automatic contouring methods. Hausdorff distance, mean distance to agreement, dice similarity coefficient, Jaccard coefficient were used to quantitatively evaluate the accuracy of the volume of contour of organs-at-risk automatically sketched by the two methods. Results:There was no significant difference in Hausdorff distance in left femoral head [(6.81 ± 2.66) vs. (7.24 ± 2.10)], right femoral head [(7.38 ± 3.91) vs. (8.14 ± 3.71)], pelvis [(24.00 ± 9.01) vs. (24.66 ± 9.67)] between AiContour ?? and Raystation ?? automatic contouring methods ( tleft femoral head = -0.831, tright femoral head = -0.821, tpelvis = -0.357, all P > 0.05). Significant differences were observed in mean distance to agreement, dice similarity coefficient and Jaccard coefficient of organs-at-risk (all P < 0.05). The mean values of dice similarity coefficient automatically sketched by AiContour ?? method were > 0.7. The DSC of left kidney, right kidney, rectum and bladder automatically sketched by Raystation ?? method were < 0.7, and the dice similarity coefficient values of other organs-at-risk automatically sketched by Raystation ?? method were > 0.7. In addition, Hausdorff distance, mean distance to agreement and Jaccard coefficient values of organs-at-risk automatically sketched by AiContour ?? method were superior to those automatically sketched by Raystation ??. Conclusion:After slight modification, the organs-at-risk automatically sketched by AiContour ?? and Raystation ?? methods can meet clinical requirement. The contouring effects provided byAiContour ?? method were superior to those provided by Raystation ?? method.

ABSTRACT

Objective To perform 3D dose reconstruction based on electronic portal imaging device ( EPID) of linear accelerator for the static intensity-modulated using Edose, a dose verification system, Aiming to assist the radiotherapy professionals to better understand the radiotherapy organs at risk and target dose changes. Methods CBCT image was acquired for patients with head and neck cancer and thoracic cancer once a week for a total of six times. Subsequently,CBCT images and planning CT images were subject to rigid registration and exported to the Edose software. According to the setup error, EPID-based three-dimensional dose reconstruction was performed by using Edose software. The gamma passing rate and dose of different organs at risk ( OARs ) were analyzed and statistically compared. Results For patients with nasopharyngeal carcinoma, the intra-fractional Dmax of the spinal cord was more significantly fluctuated and higher compared with the planning dose, whereas the intra-fractional Dmax of the brainstem did not significantly fluctuate. The V30 of the parotid gland significantly changed with a maximum increase of 28. 69% per fraction. For patients with thoracic tumors, the Dmax of the spinal cord was slightly changed, and the actual doses in the lung and heart were higher than the planning doses. The average deviation of the pulmonary V5 was up to 16. 99% between the actual and planning doses with statistical significance ( P<0. 05).According to the analysis of gamma passing rate, significant dose changes occurring in the OARs were detected in the 16th fraction for the head and neck cancer and the 24th fraction for the thoracic neoplasms. Conclusions The dose changes in the OARs can be obtained by reconstructing the EPID-based 3D dose distribution using the Edose software for each fraction, which can better protect the OAR, enhance the coverage of target dose and provide certain reference for dose-guided and self-adaptive radiotherapy.

ABSTRACT

@#Objective To observe the effects of electroacupuncture combined with edaravone on the conduction velocity of sciatic nerve and oxidative stress in rats with diabetic peripheral neuropathy. Methods 60 Sprague Dawley (SD) rats were included. 10 of them were selected as normal group. The other rats were modeled as diabetic peripheral neuropathy with streptozotocin. 48 of them were randomly selected and divided into electroacupuncture group (n=12), edaravone group (n=12), electroacupuncture + edaravone group (n=12), and model group (n=12). The threshold temperature for wave tail was tested, the levels of superoxidase dismutase (SOD) and malonaldehyde (MDA) were determined, and the conduction velocity of sciatic nerve were measured, before, and 4 and 8 weeks after modeling. Results 8 weeks after modeling, the conduction velocity and SOD increased in the electroacupuncture group, edaravone group and electroacupuncture+edaravone group compared with the model group (P<0.01), with the MDA decrease (P<0.01), while it was improved more in the electroacupuncture+edaravone group than in the electroacupuncture group or the edaravone group (P<0.01). Conclusion Both electroacupuncture and edaravone can inhibit oxidative stress and improve nerve conduction velocity of the sciatic nerve in rats with diabetic peripheral neuropathy, and it is more effective of combination.

Objective To design technique of local flap transposition to refine the aesthetic appearance of reconstructed fingers by toes transfer. Methods Nine cases with 21 reconstructed finger were included, which involved 6 males and 3 females with an average age of 21.8 years(range,18-34years).A lingual contour flap with a lateral pedicle Was shifted from the inflated distal pulp to the narrow middle part of the"finger"to refine the aesthetic appearance.Overall results were evaluated in terms of the survival of the flap, the appearance improvement and the functional influence of the reconstructed finger. Results All of the flaps survived and healed perfectly. After a mean follow-up of 9.3 months(range,6-12months),the appearance of the reconstructed fingers were impmved apparently. There was little influence on the function of the finger. The results showed that all the patients gained more acceptable fingers. Conclusion From our experience, local flap transposition is a useful method for remolding of reconstructed fingers by toes transfer.

ABSTRACT

Leptin, a cytokine predominantly secreted from fat tissue, plays an important role in regulating organism energy balance. Leptin can stimulate lipolysis, but the mechanism is unclear. In order to study the molecular mechanism of leptin stimulating lipolysis, we systemically studied the mRNA expression of key lipolytic enzymes. Morphological observation, Oil Red O staining and RT-PCR were used to identify pig primary adipocytes; commercial kits were used to measure the glycerol and FFA release; Semiquantitative RT-PCR was used to detect the mRNA expression of key lipolytic enzymes. The results showed that 100 nmol/L leptin up-regulated the mRNA expression of ATGL, TGH-2, HSL, MGL and LPL (P<0.01), but down-regulated the Perilipin mRNA expression (P<0.01). At the same time, leptin promoted the glycerol release in a dose dependent manner (P<0.01), but had no effect on the FFA release (P>0.05). These indicate that leptin may mainly stimulate lipolysis in pig primary adipocytes by up-regulating the expression of ATGL, MGL, LPL and down-regulating the expression of Perilipin. The unchanged FFA release may be resulted from Leptin promoting UCPs mRNA expression and increasing FFA expenditure.

ABSTRACT

Objective To study expression of extracellular signal-regualated kinase in intraepithelial neoplasia, prostatic cancer and BPH tissue. Methods Expression of extracellular signal-regualated kinase (ERK) was determined in 12 prostatic intraepithelial neoplasia (PIN),11 prostate cancer and 16 benign prostatic hyperplasia specimens.ERK expression was determined by immunohistochemistry. Results ERK expression was positive in all the 12 PIN specimens,6 of which being overexpressed. In the 11 prostate cancer specimens, overexpression was observed in only one while the other 10 under expressed.Overexpreassion was not observed in all the BPH specimens whereas 12 of the 16 showed underexpression. Conclusions ERK expression is obviously higher in PIN tissue (P
